Position Summary: 

The Population Health Nurse (RN) has a passion for preventing disease, promoting healthy behaviors, promoting healthy aging, and reducing health disparities. The Population Health Nurse (RN) leverages the Medicare Wellness Visit to develop a personalized prevention plan for each patient that promotes good health through disease prevention and detection. Other responsibilities include transitions of care and advance care planning. The Population Health Nurse (RN) will work in active partnership with physicians, office staff, administration, and other health care providers to develop and implement a well-coordinated plan of care to address patient needs across the continuum and ensure St. Luke’s successfully meets quality performance, enhances patient outcomes, and optimizes resource utilization. Responsible for promoting teamwork with all members of the healthcare team. Performs duties in a manner consistent with St. Luke’s mission and values. 

 

Education, Experience, & Licensing Requirements:   

Education: Nursing Degree   

Experience: 3 Previous years of clinical experience  

Licensure: Current licensure as a Registered Nurse in the state of Missouri
