Position Summary:
The Population Health Coordinator is responsible for coordinating patient care activities for patients in need of enhanced care coordination. The Population Health Coordinator will ensure patients receive timely and high quality healthcare and support services through the use of care protocols, care plans, self-management goals and referrals to the most appropriate resources. Responsible for promoting teamwork with all members of the healthcare team. Performs all duties in a manner consistent with St. Luke’s mission and values.
Education, Experience, & Licensing Requirements:
Education: Associate Degree or Healthcare Credential (LPN, RN, etc.)
Experience: 3 Years of Healthcare Experience
