Position Summary: 

St. Luke’s Heart and Vascular Institute utilizes a program coordinator navigator to assists in driving  quality-based, patient-focused program. The  CardioCare Coordinator is a clinical professional with a deep understanding of Cardiovascular Disease and specifically, Structural Heart Disease, who is devoted to managing program implementation, execution, and ongoing maintenance.

 

Under the supervision of the cardiac service quality manager, provides professional care to patients with structural heart disease. Responsible for coordinating optimal patient and family experiences from diagnosis, referral to an appropriate CV Specialist, evaluation, treatment, and follow-up for patients with structural heart disease. Works cooperatively and communicates effectively within a multidisciplinary framework to actively promote care for patients. Demonstrates self-direction and takes initiative in an active role as a care coordinator to achieve set quality improvement goals. 

 

Education, Experience, & Licensing Requirements:   

Education: Nursing Degree    

Licensure: Current licensure as a Registered Professional Nurse in the State of Missouri.  

Experience: 2 years of Cardiac Clincal RN experience, strong knowledge of cardiovascular and structural heart disease
