Position Summary: 

Assumes responsibility of Team Leader for the Interdisciplinary Care Plan Team to include in meeting Federal and State Regulations related to Resident/Client Care Plans and MDS. Performs duties and responsibilities in a manner consistent with our mission and values. Responsible for promoting teamwork with all members of the healthcare team. Performs all duties in a manner consistent with St. Luke’s mission and values. 

 

Education, Experience, & Licensing Requirements:   

Education: Graduate of an accredited Nursing Program 

Licensure: Current licensure as a Registered Nurse in the state of Missouri
